OMC to offer free athletic physicals for area students

Ozarks Medical Center will offer free athletic physicals for area student athletes from 4:30 to 8 p.m. Thurs-day, July 19, at the Shaw Medical Building, across from the main entrance of the hospital at 1100 Kentucky Avenue in West Plains.
Participants are encouraged to arrive during these specific time slots, determined by the first letter of their last name:
4:30 to 5 p.m. A to E
5 to 5:30 p.m. F to I
5:30 to 6 p.m. J to M
6 to 6:30 p.m. N to Q
6:30 to 7 p.m. R to U
7 to 7:30 p.m. V to Z

Local physicians, family nurse practitioners and advanced practice nurses will conduct the physicals for student athletes entering 7th through 12th grades in local area schools. 
Athletic physicals are required for all students who plan to participate in any school-sponsored sport.
Last year, 320 area students turned out for this event.
All students will be required to submit a signed consent form and current insurance information from a parent or legal guardian at the time of the physicals. Forms must be completely filled out in order for the student to receive the physical. Partial information and incomplete forms will not be accepted.
Consent forms may be picked up between 8 a.m. and 4 p.m. Monday through Friday at the West Plains High School office, at OMC Education Services in the Parkway Center in West Plains or at OMC Rehabilitation Services on the second floor of the Shaw Medical Building.

Boys requesting physicals should use the Kentucky Avenue (east side) entrance of the Shaw Medical Building. Girls should use the main Shaw Medical Building entrance.
